Paternity Leave

  1. Please ensure that you have read the All About Healthcare Paternity Leave Policy, before completing this form.
  2. Please note that any public holidays, which fall during the period of maternity leave, will be added on to the end of the period.
  3. If you intend taking annual leave directly following paternity leave, this must be agreed in advance with your Manager.
